Amendment

Per OGC, appeal of final order of Historic Preservation Commission denying minor modification of Certificate of Appropriateness for roof reconfiguration on Shadowlawn Street amended to grant the appeal based on substantial changes in circumstance as evidenced by testimony from a roofing contractor and architect about the unwarrantability of the roof structure as originally approved.

Per OGC, rezoning on Pearl Street to PUD approved with conditions: any new structure shall be set back a minimum of 40 feet from the easterly property boundary and an minimum of 25 feet from the northern property line of the historic Drew Mansion property; front porches shall be no closer than 8 feet from a front property line; HVAC equipment shall not be located within any front yard setback; all on-site signage shall be consistent with the requirements of the Riverside/Avondale Zoning Overlay; applicant shall install aesthetically appropriate fencing subject to review and approval by the Historic Preservation Commission where adjacent to single-family or duplex residential properties except in the narrow north/ south portions of the public alleys; there shall be no outdoor audio speakers on the property in connection with any proposed commercial use; applicant shall install pedestrian scale parking lot lighting as depicted in the Jacksonville Design Guidelines and Best Practices Handbook or as otherwise approved by the Historic Preservation Commission; a minimum 85% opaque, 8-foot high screen shall be provided to block the view of the pool amenity with materials and design reviewed and approved by the Historic Preservation Commission; a minimum off-street parking space to bedroom ratio of 1 to 1 shall be provided regardless of unit type. Per OGC, rezoning on Tyson Lake Drive from PUD to PUD approved with conditions: signage shall be erected by developer that indicates on-street parking shall be limited to one side of the street only; development shall work in good faith to provide a connection to the future extension of New World Avenue; there will be no increase in density from the currently-approved PUD; the new 50 foot lots will have the same front, side and rear yard setbacks as required by the existing PUD; the same floor plans and elevations as currently approved by the Architectural Review Board will be used unless otherwise approved by the ARB; the developer will diligently pursue completion of Unit 5 of Adam’s Lake as currently approved under the existing PUD; the developer will create 2 new passive recreation areas as denoted on the attached site plan – Area A is a small upland which will be accessed from a city street to be named and Area B will encompass the land around a lake and will include a mulch walking trail; developer will provide a mulch walking trail to the property line contiguous to the School Board property on land which is part of Adam’s Lake Unit 6; developer will make a contribution of $15,000 to the Adam’s Lake Owners Association to be used for recreational improvements to be determined by the HOA board; developer will provide for traffic signalization of the intersection of Normandy Boulevard and Adam’s Lake main entrance; developer shall provide a 60-foot right-of-way as depicted on Revised Exhibit 4 to be reserved for a period not to exceed 5 years from the date of adoption of this rezoning; in the event the 60-foot ROW is not improved by the adjacent property owner to the east, 50 feet of this area shall revert back to the developer for use as a single family lot (44b) in the development, bringing the number of lots to 102; in the event of this reversion occurring, the developer shall reserve a 10-foot ROW for pedestrian access to future development to the east. Per OGC, waiver of minimum road frontage on Wycoff Avenue approved subject to conditions: there shall be no further subdivision of this property beyond the proposed 2 lots as shown on the attached site plan; the easement as noted on the site plan shall be recorded with the Clerk of the Courts. Donation of surplus property to Kairos Development International: removes certification of surplus of property, because the property is already declared surplus by virtue of being on the Affordable Housing Inventory List;  authorizes the transfer of the property pursuant to the Donation Agreement terms;  removes reference to authorization of conveyance in accordance with Subpart B Ch. 122, because with the exception of the sections being waived, the conveyance will be in compliance with Subpart F; attaches a donation agreement.
