Substitute

(Approp $37,398.25 from Tree Protection & Related Expenses Trust Fund for Patton Rd & Beach Blvd Tree Planting):

1.      Changes total appropriation amount to $94,283.36 ($62,080.49 from Charter Subfund 15N and $32,202.87 from Ord. Subfund 15F)

2.      Changes planted trees from Charter Fund to the following:

a.       31 Southern Magnolia

b.      37 Southern Red Cedar

c.       19 Longleaf Pine

d.      8 Live Oak

3.      Changes planted trees from Ord. Fund to 6 Yaupon Holly Trees

4.      Includes irrigation, fertilization and an additional year of maintenance as part of the cost of the tree planting

5.      Attaches a Revised Exhibit 1 (revised BT)

6.      Attaches a Revised Exhibit 2 (revised Tree Planting Plan).

Amendment

(Rezoning 6664 & 6692 Firestone Rd btwn Morse Ave & 118th St. from RR-Acre to PUD): Per OGC, rezoning granted subject to conditions:

(1) Personal property storage facilities shall comply with Part 4 of the Zoning Code.

(2) The developer shall provide a thirty (30) foot front yard along Firestone Road. Internal vehicle-use-area landscaping may be relocated to the front yard for supplemental screening, provided that the total landscape planting requirement is not diminished.

(3) Walls facing Firestone Road shall be of masonry construction.

(4) Prior to the first final inspection within any phase of development, the owner or their agent shall submit to the Planning and Development Department for its review and approval either (a) an affidavit documenting that all conditions of the development order have been satisfied, or (b) a detailed agreement for the completion of all conditions of the development order.

Amendment

(Rezoning 7981 Baymeadows Cir W. btwn Baymeadows Cir W. & Baymeadows Circle E.  from PUD to PUD): Per OGC, rezoning approved subject to conditions:

(1) The Subject Property shall be developed in accordance with the Development Services Division Memorandum dated February 24, 2014, attached hereto as Exhibit 6, or as otherwise approved by the Planning and Development Department.

(2) A right turn lane shall be installed along Baymeadows Road from Baymeadows Circle East to Baymeadows Circle West.

(3) The applicant shall apply to FDOT to close the median on Baymeadows Road between Baymeadows Circle East and Baymeadows Circle West.

(4) The driveway access on Baymeadows Road shall be right-in, right-out only.

(5) A permanent accessway connecting Baymeadows Circle East and Baymeadows Circle West shall be constructed along the north property line.

(6) Where adjacent to the Subject Property, Baymeadows Circle East and Baymeadows Circle West shall be improved to City roadway standards, or as otherwise approved by the Planning and Development Department.

(7)  Development shall be generally consistent with the conceptual renderings dated August 20, 2018, attached hereto as Exhibit 5.

(Amend Chapt 82 (Jacksonville-Duval County Council on Elder Affairs), Ord Code, to Clarify Mbr Composition; Clarify and Amend the Selection Process for the Exec Dir; Clarify Meeting Locations; and to Amend the Duties of the Council): corrects scrivener’s errors; adds strikethrough to existing language being struck.

(Apv Amend One to Redev Agreemt with Jacksonville Historical Society, Inc): Replaces On File  document with Revised On File document to add language in the redevelopment and repayment obligation security agreements clarifying that if the JHS continues to utilize one or both of the buildings for their primary office, as the primary location for its historical archives, or for other uses consistent with the organization’s mission that the grant will not need to be repaid.

(Amend Chapt 70 (Duval County Tourist Dev Council), and Chapt 666 (Duval County Tourist Dev Plan), Section 666.108 (Tourist Dev Plan), Ord Code; Apv Tourist Dev Council Grant Guidelines):

 

NCSPHS:

1.Adds a definition for TDC Signature Events and excludes these events from the multiple years funding concept

2.Inserts the funding amount of $20,000 for the Equestrian Center Plan Component in the Funding Allocations for Plan Components chart

3. Attaches  Revised Exhibit 1 (revised TDC grant guidelines)

 

Finance:

NCSPHS amendment, plus adds language that Equestrian Center grants shall be awarded in accordance with TDC Grant Guidelines approved by City Council; attaches Second Revised Exhibit 1 (second revised TDC grant guidelines)

Substitute and rerefer

(Amend Chapt 70 (Duval County Tourist Dev Council), Sec 70.105 (Admin of Tourist Development Plan), Ord Code; Amend Chapt 106 (Budget and Acct Code), Pt 7 (Travel Expense Reimbursement):

1.      Corrects scrivener errors  in legislation and Exhibit 1

2.      Includes “incidental expenses” in the definition of Travel Expenses to mirror Florida Statutes

3.      Adds language that requires the travel schedule be included in the contractor’s annual plan approved by the TDC

4.      Adds language that the TDC, in its discretion, may only advance pay or reimburse the Travel Expenses in accordance with the annual travel budget and annual travel schedule approved by the TDC and that any amendments will require TDC approval

5.      Clarifies that any rules that the TDC develops regarding Travel Expenses must be consistent with tourism industry practices

6.      Adds language that food and beverage expenditures must also be consistent with annual budget and annual plan approved by TDC.

Amendment

(500,000 from Spec Council Contingency - Police Athletic League to the Police Athletic League Trust Fund for Improvemts to Police Athletic League Facilities and to Replace Two 16 Passenger Buses):

1. Corrects passenger capacity of buses being purchased

2. Adds reverter language to the legislation if buses are not used for intended purpose or if PAL wants to sell the buses within a 5-year period

3. Attaches Revised Exhibit 1 (revised BT)

4. Adds language in the Agreement with PAL to address the following:

- Require that they obtain 3 quotes for all the repairs and purchases related to the $470,000 provided by the City

- Require that a separate audited schedule on the $470,000 be included with their audited financials

- Provides the Council Auditor’s Office with audit rights to all records related to the $470,000 grant.

Amendment

(Declaring that Public Convenience & Necessity Require Issuance of a Certificate for Emerg Medical Transportation Service to Kindred Care EMS, LLC):

1. Revises On File Document to correct company name

2. Updates language on 2018-410 and 2018-411 to reflect their approval by Council

3. Updates waiver section to reflect there are currently five certificates issued and this would be the sixth; to add language that one certificate can be issued for every 200,000 people; and strike reference to up to four certificates.

4. Includes condition to issuance of COPCN that company agrees to sign an MOU.
