(Authorizing the Employee Services Department to Offer the Option to Enroll in a UF Health Plan to City Employees or Retirees): changes the bill from an authorization for the Mayor to negotiate and execute a third-party administrative services contract with Integra Administrative Group to a bill approving an attached contract.

Amendment

(Approving Waiver of Minimum Road Frontage Application on Townsend Rd.): Per OGC, approves waiver subject to conditions: 1) If the land is ever subdivided, the access to the property shall be brought up to City standards, and a document so stating, approved by the Office of General Counsel, shall be filed in the public records of Duval County.

2) Prior to requesting a final building inspection or occupying the facility in any manner, the developer or owner shall submit to the Planning Department a copy of the recorded restriction placed on the property regarding the requirement for the access to be brought up to City Standards.

Amendment

(Authorizing 2nd Amendment to Development Agreement with Baptist Health Properties for Project Formerly Known as "Plaza North"):  clarifies that Baptist Health Properties, Inc. is the “transferee”; corrects the committee that held the required public hearing to TEU; revises the term extension limitation date to June 22, 2022; attaches Revised Exhibit 1 (revised agreement).

Amendment

(Rezoning 5901 Sheffield Rd from AGR to PUD): Per OGC, approves the rezoning subject to conditions: 1) The development of the entire 7.46 acre site shall be limited to no more than one single-family residence, and a document so stating, approved by the Office of General Counsel,  shall be filed in the public records of Duval County.

2) Prior to requesting a final building inspection or occupying the facility in any manner, the developer or owner shall submit to the Planning Department a copy of the recorded restriction placed on the property regarding the limitation to the development of only one single family residence.

Corrects scrivener’s error in a parcel RE number; attaches Revised Exhibit 4 (revised site plan).

Amendment

(Rezoning Property on Normandy Blvd at Bivins Rd from CCG-1 to PUD): Per OGC, rezoning approved subject to conditions: 1. There shall be a maximum of one ground sign on Normandy Boulevard not to exceed 160 square feet in area and 30 feet in height. 2. All sag lenses, drop lenses and convex lenses shall be prohibited. Illumination levels at all property lines shall not exceed one-half (.5) foot candles (“f.c.”) when the building or parking areas are located adjacent to residential areas, and shall not exceed one (1.0) f.c. when abutting other non-residential properties. All lighting lamp sources within parking and pedestrian areas shall be metal halide, compact fluorescent or LED. The maximum light pole height in all parking areas should not exceed thirty feet (30’- 0”). An exterior lighting design plan, including a photometrics plan, pole and fixtures schedules shall be submitted at the time of Verification of Substantial Compliance for review and approval by the Planning and Development Department. 3. The developer shall mount light fixtures on canopies so that the lens cover is recessed or flush with the bottom surface (ceiling) of the canopy. 4. New construction shall comply with Chapter 656, Part 12 Landscape and Tree Protection Regulations of the Zoning Code. 5. Prior to requesting a final building inspection or occupying the facility in any manner, the owner, applicant or developer shall submit to the Planning Department separate certification letters confirming that all horizontal and vertical components of the development have been substantially completed, and all conditions to the development order have been satisfied. This condition shall apply to both phased and non-phased developments.

(Appropriating $144,500 from Special Law Enforcement Trust Fund to Fund Crime Prevention Activities): attaches a new Exhibit 3 (funding allocation information).

(Appropriating $312,500 from General Fund/GSD Fund Balance to the JTA for a Capital Project Related to the St Johns River Ferry): removes CIP amendment language as the Ferry is not a City asset; removes Exhibit 3 (CIP page).

(Authorizing Franchise Agreement with Buckeye Terminals, LLC for a Liquid Energy Ethanol & other Petroleum Transmission Line)

 

TEU: strikes waiver of all of Chapter 711, Part 3 and instead waives Sections 711.306(a), 711.307, 711.319 and 711.321; attaches Revised Exhibit 1 (revised agreement) to correct payment schedule; includes fund deposit account information.

 

Finance: TEU amendment, plus adds Whereas clauses regarding uniqueness of the project and need for use of public right-of-way; attaches Second Revised Exhibit 1 (second revised agreement with new insurance requirements).
